Setting up inspection questions
Who is this article for?
Users who need to setup Questions.
Administrator role is required.
Inspection questions define what is assessed, how responses are handled, and what actions are generated for unsafe observations.
1. Creating questions
To create a new inspection question, follow these steps:
- Navigate to Edit Inspections.
- Select Questions.
- Use filters to find existing questions or click + New Question to create one.
- Configure the core fields as described in the table below.

| Requirement Group | Determines who is responsible and if corrective actions are generated:

2. Configuring optional fields
You can configure additional optional fields to enhance question functionality:
| Summary Corrective Action | Text displayed in aggregated email; defaults to Comment/Action if blank |
| Comment Link | Add hyperlink to Comment/Action |
| Action Question | Additional fields triggered when corrective action is generated |
| Location-Based Action | Customise action text per location |
| Safe Comment / Safe Comment Link | Optional message and link for safe observations |
| Action Tags | Predefined tags for organising corrective actions; editable during inspection |
| Resource / Resource Link | Reference notes or hyperlinks in the inspection report |
3. Setting up parent–child questions
Parent–child questions allow you to trigger child questions based on parent responses.
To set up parent–child questions, follow these steps:
- Create a parent question with Requirement Group set to None.
- Set answer options to Yes or No.
- Create the child question as usual.
- Find the child question in the list.
- Click the blue parent icon.
- Select the parent from the dropdown.
- Parent and child questions appear together in templates.
